Required Forms

Outstanding care of our campers, both physically and emotionally, is our number one priority at America's Camp and the enclosed forms are an essential part of providing that level of care. All forms must be submitted for an enrolled camper to participate this summer.

Start with the Forms Guide below. The guide includes details about the purpose of each form and how it should be filled out. For some forms, only one per family needs to be completed. For others, one form must be filled out per camper. Please note that some forms can be completed by a parent/guardian, while others must be completed by the child's doctor. In addition to the forms below, please send a photocopy of each child's insurance card.
